Wallet card: Raise a concern with my support worker
Print these cards, cut them out, and laminate them so you can carry the words with you and show or read them when you need to.
How to print your wallet card
- Press Print / Save as PDF above.
- Set Scale to 100% (Actual size) — not “Fit to page” — so the cards stay wallet-sized.
- Turn on Background graphics (under “More settings”) so the colours print.
- Use thick paper or cardstock(about 200–300 gsm) if you have it. Plain paper works too.
- Cut along the dashed lines.
- Laminate each card, or cover both sides with clear tape, so it lasts. Keep them in your wallet or bag.
